Jagatsinghpur:- Irregularities in Birth & Death Certificate has became a major issue in Jagatsinghpur district. Particularly massive irregularities found at CHC , Kujang and special enquiry started by Dr.Ajay Kumar Dalai, Addl Director cum Addl Chief Registrar of HI & VS Odisha on 7 th May 2026. As per letter no. 938/23/4/26 of Director of Health Intelligence & Vital Statistics, Odisha the Registrar Birth & Death cum Medical Officer – in – charge , CHC, Kujang was intimated to provide documents for verification. As per information from Bench clerk of Vital Statistics more than 1000 birth and Death certificate prepared upon fake documents and randum irregularities found.
Basing upon the report of investigating Officer Dr. Ajay Kumar Dalai Chief District Medical Officer , Jagatsinghpur issued letter no. 436/26/5/26 to Registrar Birth & Death cum Medical Officer, Kujang to provide all documents to special investigating team on 3rd June 2026 . Even Medical Officer directed to present there with accountant and Bench Clerk of Vital Statistics with records of Kujang CHC from 18/7/2025 to 14/1/2026. Though enquiry by higher authorities completed but Kujang CHC authorities and Chief District Medical Officer not providing clear data.
As per information of Kujang CHC staff about 1000 above Birth and Death certificate prepared upon fake data in a irregular manner and more than one crore rupees taken as kick back by the Vital Statistics clerk and Registrar Birth & Death cum Medical Officer, CHC, Kujang. General people demand high level enquiry report should be open up and District Magistrate should take immediate action against the miscreants.
